Throughout the past 20 years, interoperability in information systems in the Portuguese National Health Service (Portuguese NHS) has been mostly limited to solutions using data link layer in the Open Systems Interconnection (OSI) model. These kind of connection creates a significant and complex problem since their implementation has high variability from sight to sight and used extensively there is a risk of loss of control over who has access to data from a given system. These cibersecurity risks are now even more relevant and linked also to privacy control challenges. This paper aims to inform about the context of the Portuguese Health IT Architecture, being rolled-out at present, the challenges that arise from years of dispersion and heterogeneity in its core information systems, and how the Local Interoperability Gateway for Healthcare (LIGHt) project came to be in providing an event driven approach to systems interoperability, standards adoption and ease of control and monitoring. |
