Paper Abstract:      This paper tackles the problematic processes of knowledge creation, transfer and use that take place in the context of medical personnel and patients and how these processes can be improved by the application of IS/IT. The concept of patient-centered approach (PCC) has been introduced as a means of creating better quality and efficiency in health care. The initiating idea of the paper is that we can contribute to PCC by introducing models and concepts from the knowledge management area. We explore this problem from the practical case situation of long-term patients. As the outcome we present a systemic knowledge management model, where important aspects and relations between the parties involved are laid out.
