Paper Abstract:
|
Despite the fact that elderly patients with congestive heart failure (CHF) represent the majority of hospitalizations among
adults over 65, little is known concerning their survival based on disease management. The numbers of CHF patients that
die within 10 years of the onset of symptoms is staggering. The objective of this project is to develop a mobile patient
monitoring, symptom recognition and self intervention system along with a complementary clinical nurse patient
monitoring and decision support tool. The goal of the patient mobile learning tool is to assist patients through simple
interactions and feedback to learn how to recognize the relative status of their health condition, to recognize and
understand symptom and status changes either positively or negatively and to develop and put into place self
interventions that can be tried in between clinical encounters with a health care professional. The nursing mobile tool
allows the clinical nurse practitioner to select a patient to examine, extract out self reporting information, combine this
with clinician entered measurements to determine the status of a patient and develop an intervention plan tailored to the
specific patients needs.
The foundation for the tool has been developed as well as a prototype implementation. The researchers are presently
developing a clinical trial for the device along with the clinical protocols within a proposed national institute of health
research project. |
