Paper Abstract:      Community nursing in Ireland refers to the delivery of healthcare to patients at several locations by nurses with different specialties. To ensure continuity of care, community nurses need to access patient-centered information collected during health assessments as the patient moves through the system. The comparison of assessments at different stages of the care process produces patient-centred healthcare evaluation measures called outcomes. The outcomes give a better visibility of the impact the care delivered by each service has on the patient’s health. However, successful information communication and comparison necessitates a high level of interoperability with the use of agreed standards, data types, and code sets. In this paper we describe the presentation of a patient assessment tool compliant with European standards (CEN EN13606, TS14796), and using terminologies describing patient outcomes (C-HOBIC) and nursing practices (ICNP). The tool is developed within the PARTNERS (Participatory Action Research project To develop Nursing Electronic ResourceS) project, a part of the EHRland (EHR for Ireland) project. The demonstration includes the capture of data from assessment forms, and the generation of data views specific to the interests of health service providers and the patient. Section 1 presents the assessment tool and Section 2 describes the demonstration.
