Paper Abstract:      As new technologies are developed and society’s expectations of healthcare change, so the delivery of healthcare itself changes. This is driven by policy commitments to provide more care outside of hospitals, closer to home and in the home, to reduce hospital stays and ensure value for money for health systems challenged by resource constraints and a growing older population. Managing chronic illness in older adults presently utilises around 70% of health services resources in Ireland. The potential of digital technology and remote monitoring to transform care and promote sustainability of the healthcare system is recognised, particularly in the management of heart failure. However, in practice little has change in how health professionals manage the care of vulnerable older adults despite long held promises that technology would transform the delivery of health care. This project explores the range of challenges associated with reshaping the Ireland’s public-sector healthcare workforce to facilitate telemedicine for older adults living with chronic illnesses. It will also contribute to knowledge on how this intervention works or not to inform policy and practice.
